Hello DCC Dolphins,

Below you will find the Swim/Dive Team Photo Order form please feel free to print (for each child) and bring with you for Tuesday's Photo day.  They accept cash, checks and credit cards. They ask that you fill out a form for each child/sibling photo that you would like to order. We will also have some printed on-site as well.

Example: If you would like Package A for Jane Jones, Package B for John Jones and an 8x10 of Jane and John Jones. You would need 3 forms. If you only want a sibling photo, it would be 1 form since you are not wanting individual of each child. 

Dive team photo is slotted for 2:45pm 

Swim team photo is slotted for 3:15pm

Individual photos will be taken in between and after.  

Please wear your team suit if you can. We look forward to seeing you there!
